In a large skillet over medium heat, crumble the breakfast sausage. Cook it until browned and fully cooked through, stirring occasionally (approximately 5-7 minutes).
Once the sausage is cooked, reduce the heat to medium-low and sprinkle the flour over the sausage, stirring to combine. Cook for 1-2 minutes to remove the raw flour taste.
Slowly pour in the milk, whisking continuously to avoid lumps. Bring the mixture to a gentle simmer, and allow it to thicken (about 5-7 minutes). Stir often to prevent sticking.
Season the gravy with salt, black pepper, cayenne pepper (if using), garlic powder, and onion powder. Adjust seasonings according to taste.
Continue to cook until the gravy reaches your desired thickness. If it gets too thick, add a bit more milk to reach the consistency you prefer.
